What do I need to bring if transferring from another practice?
You must register with our office as a new patient, please call us at 859-286-9046. Please let the receptionist know that you are wanting to register new patient. If you need to schedule an appointment, the staff member can also do that when you call to register with our office. We will do our best to promptly see new patients for well child physical exams and routine care of chronic issues. We also offer same-day availability for acute care appointments (sick visits).
You will also need to make arrangements for your child’s previous medical records to be sent to us. When transferring your child’s care from another practice, we will ask that you fill out a medical records release at our front desk or you can download the form from our website. You can drop off the completed form at our office or fax it to us so that we can request prior medical records for you. We require for the medical records to be released to our office within 90 days of the child's first visit. Please note that you must complete a release form for each child and each practice where your children were previously seen. We do not accept medical records by fax, which means they need to be mailed or you can bring them with you to your child's appointment.
We are expecting our first child, do you offer a meet and greet with your provider?
We do offer a prenatal consult with our provider. The consultation fee is $50.00 and is not billed to insurance. If you are interested, please call our office at 859-286-9046 to schedule a 10-15 minute consultation.
If I need a referral do I need an office visit?
Yes, our provider will need to have an office visit with you to make a referral. Please make an appointment to be seen.
How do I make a payment?
We do not accept payment over the phone. You may pay your balance by mailing in cash or a check, or by coming in the office to pay any balance on your child(s) account.
My child is scheduled for a well child checkup, can I discuss new or existing concerns, worries, or symptoms at this visit?
For anything other than the well child checkup, we highly recommend you to schedule an office visit. This will allow our provider more time with you to go over any questions you may have.
If any concerns are addressed during the well child checkup, an office visit may be billed which will result with the patient having a copay.
This includes any referrals, prescriptions, and lab/imaging orders.
